Palatal Expander
Upper jaw widening device

What it does

A palatal expander widens the upper jaw (maxilla) to create more space for teeth, improve the bite, and expand the airway. It works best in growing patients because the two halves of the palate have not yet fused.

How it works

The expander is bonded to your upper molars and has a small screw in the center. Activating the screw a tiny turn each day (or as prescribed) gradually separates the two halves of the palate, allowing new bone to fill in the gap. The process is slow and painless.

Wear Instructions

  • Activate the expander exactly as directed — your instructions will tell you how many turns per day and for how many days.
  • Use the key provided: insert it into the hole, push firmly toward the back of the mouth to rotate the screw, and remove the key.
  • A small gap between the upper front teeth is normal and expected — it will close on its own after expansion is complete.
  • Wear the expander full time. It is only removed by Dr. Oliver.

How to Clean

  • Rinse vigorously with water after every meal to flush food from under the expander.
  • Use a water flosser or a syringe with water to flush the area directly under the expander where the tongue cannot reach.
  • Brush around all the metal bands and the central screw with a soft toothbrush.
  • Avoid sticky or chewy foods — they get trapped under the expander and are very difficult to clean out.

What to Expect

You may experience mild pressure or soreness after each activation — this is normal and usually passes within a few minutes. Speech may be slightly affected at first but improves quickly. A gap between the front teeth is normal and expected during active expansion.

When to call us: Call us if you notice the expander feels loose, if there is significant pain rather than mild pressure, or if you lose the key.

Herbst Appliance
Lower jaw positioning device

What it does

The Herbst appliance corrects overbites (where the upper teeth and jaw protrude significantly beyond the lower) by positioning the lower jaw forward during growth. It is a fixed appliance worn 24 hours a day.

How it works

Metal tubes and rods connect the upper and lower jaws in a slightly forward position. This constant gentle positioning encourages the lower jaw to grow forward while restraining excess growth of the upper jaw. It is most effective in growing patients.

Wear Instructions

  • The Herbst appliance is fixed and cannot be removed — it works around the clock automatically.
  • Avoid very hard or crunchy foods — biting into hard things can dislodge or bend the rods.
  • Open your mouth wide slowly when yawning or biting into large foods — do not force the mechanism.
  • Return for all scheduled adjustment appointments as scheduled — these are critical to treatment progress.

How to Clean

  • Brush around all metal components thoroughly after every meal.
  • Pay special attention to the area where the tubes connect to your molar bands.
  • Use a proxy brush (interproximal brush) to clean under and around the rod-and-tube mechanism.
  • A water flosser on medium pressure works well for flushing the appliance.
  • Floss using a floss threader to get between all teeth.

What to Expect

Your cheeks and tongue need a few weeks to adapt to the hardware inside your mouth. Mild jaw soreness is normal initially. You may notice changes in how you chew — softer foods are recommended during the adjustment period.

When to call us: Call us if a tube or rod comes loose, if there is significant jaw pain rather than mild soreness, or if the appliance appears bent or broken.

Headgear
Jaw growth guidance appliance

What it does

Headgear applies gentle force to guide the growth of the jaw and facial bones in growing patients. It is most commonly used to restrain upper jaw growth in patients with a significant overbite, or to help close spaces and move back teeth.

How it works

Headgear attaches to hooks on your upper molar bands via a facebow (a U-shaped wire), and connects to a strap that wraps around the back of your head or neck. The force from the strap gently holds the upper jaw from growing too far forward relative to the lower jaw.

Wear Instructions

  • Wear your headgear for the number of hours prescribed by Dr. Oliver — typically 12–14 hours per day, usually during sleep and evening hours.
  • Always remove headgear before playing sports, exercising, or doing any physical activity.
  • Never wear headgear when rough-housing or around young children.
  • Insert and remove carefully — do not yank or pull the facebow out quickly.
  • Wear consistently. Inconsistent wear dramatically reduces effectiveness and can extend treatment time.

How to Clean

  • Wipe down the facebow with a damp cloth after wearing.
  • Clean the strap or headcap with mild soap and water as needed.
  • Brush your teeth and clean your molar bands thoroughly — headgear attaches to these bands so they need extra attention.
  • Store headgear in its case when not in use.

What to Expect

Some pressure and mild soreness when first wearing headgear is normal. This typically passes within a few days. You will adapt quickly, and most patients find it easy to sleep with headgear on after the first week.

When to call us: Call us if the facebow bends, if the molar bands feel loose, or if you experience unusual pain or discomfort.

Twin Block Appliance
Removable jaw repositioning device

What it does

The Twin Block is a removable functional appliance that positions the lower jaw forward to correct overbites and encourage proper jaw growth. It consists of upper and lower plates with angled blocks that fit together when the mouth is closed.

How it works

When you bite down, the angled blocks on the upper and lower plates guide the lower jaw into a forward posture. Worn consistently during growth, this trains the jaw muscles and stimulates bone growth in the lower jaw while restraining the upper.

Wear Instructions

  • Wear the Twin Block for the hours prescribed — Dr. Oliver will specify your schedule.
  • Wear it while sleeping, during school, and as much during the day as possible.
  • Remove it to eat and to play contact sports.
  • When eating, carry your case so you can store it safely during meals.
  • It may feel awkward at first — your speech will adapt within a week or two. Reading aloud accelerates this.

How to Clean

  • Remove and rinse under cool water after meals.
  • Brush both plates gently with a soft toothbrush and mild dish soap or denture cleaner.
  • Do not use toothpaste — it can scratch the acrylic surface.
  • Soak in a retainer cleaning tablet a few times per week.
  • Store in the hard case provided whenever the appliance is not in your mouth.

What to Expect

Initial soreness in the jaw and teeth is common during the first 1–2 weeks as your muscles adapt to the new jaw position. Speech changes at first are normal — you will sound more normal quickly. Stick with it: consistency is everything with functional appliances.

When to call us: Call us if the appliance cracks, if a block or wire comes loose, or if you experience significant pain rather than the normal adjustment soreness.

Rubber Bands (Elastics)
Bite correction elastic bands

What it does

Elastics are small rubber bands worn between hooks on your upper and lower braces or aligners to correct the relationship between your upper and lower teeth — improving bite alignment, fixing overbites, underbites, and crossbites.

How it works

By connecting upper and lower teeth in specific patterns, elastics apply continuous gentle force that moves your jaws and teeth into proper alignment. The specific hooking pattern and elastic size are prescribed by Dr. Oliver based on your bite needs.

Wear Instructions

  • Wear your elastics exactly as prescribed — Dr. Oliver will show you the correct hooking pattern at your appointment.
  • Wear them 24 hours a day, 7 days a week, removing only to eat and brush your teeth.
  • Replace elastics with a fresh pair after every meal — used elastics lose their force.
  • Always put new elastics in immediately after brushing. Never go to bed without them.
  • If an elastic breaks, replace it right away — even one missed hour adds up over weeks.
  • Carry extras with you at all times — we provide bags of them, and they are free at our office.

How to Clean

  • Remove elastics before brushing and flossing — you cannot clean around them properly with them in.
  • Replace with a fresh elastic immediately after brushing.
  • Brush around all hooks on both upper and lower brackets where elastics attach.
  • Check that hooks are clean and free of plaque buildup at each brushing.

What to Expect

Your jaw and teeth may feel sore for a few days when you first start wearing elastics, and again after each change in pattern. This is normal. You may also notice some jaw fatigue initially — this passes quickly. The results from consistent elastic wear can significantly shorten your overall treatment time.

When to call us: Call us if you run out of elastics, if you are unsure of your hooking pattern, or if you notice a hook on a bracket has broken off.

Space Maintainer
Tooth space preservation device

What it does

A space maintainer holds open the space left by a prematurely lost baby tooth. When a baby tooth is lost too early, neighboring teeth naturally drift into that space — preventing the permanent tooth from erupting properly. A space maintainer preserves the space until the permanent tooth is ready.

How it works

Space maintainers are typically fixed metal bands and wires bonded to teeth on either side of the gap. They hold the space open passively — the appliance does not move teeth, it simply stops adjacent teeth from moving into the space.

Wear Instructions

  • Fixed space maintainers cannot be removed at home — they work automatically.
  • Avoid very sticky or hard foods that could dislodge or bend the appliance.
  • If the appliance feels loose or comes off, contact our office right away — teeth can drift very quickly.
  • Dr. Oliver will monitor the permanent tooth's progress at each visit and remove the maintainer when the tooth begins to erupt.

How to Clean

  • Brush thoroughly around all bands and wires at least twice a day.
  • Pay special attention to where the metal band meets the gum — plaque builds up here easily.
  • Use a proxy brush to clean around the wire on both sides of the gap.
  • Floss around the appliance using a floss threader.
  • Rinse with a fluoride mouthwash to protect the enamel around the bands.

What to Expect

A space maintainer should not cause significant discomfort after the initial brief placement appointment. Your child's tongue may play with the wire at first — this is completely normal and passes quickly.

When to call us: Call us immediately if the appliance comes loose, feels sharp, or if your child is having difficulty eating. Time is critical when a maintainer comes off — teeth drift fast.

Tips for Every Appliance Patient

🦷

Hygiene is critical — appliances create many extra surfaces for plaque to hide. Brush and rinse after every meal.

🚫

Avoid the foods on the foods-to-avoid list — hard and sticky foods are especially damaging to appliances.

📋

Bring your appliance (if removable) to every appointment, even if you think it looks fine.

📞

If something feels wrong — loose, broken, or poking — call us rather than waiting for your next scheduled visit.

💬

Speech adapts. Most patients are speaking normally within one to two weeks of getting a new appliance.

💊

Soreness at first is normal. Over-the-counter pain relievers and soft foods help during the first few days.

Something Does Not Look Right?

Never wait when something seems off with your appliance. Most issues are simple fixes — but only if we catch them quickly
